As a reminder, the Connect4Health Premium Incentive Program has been suspended for the 2019-2020 year due to the COVID-19 pandemic.
What this MeansA preventive exam (annual physical, mammogram and colonoscopy or well-woman exam) plus a biometric screening is not required of employees, spouses or domestic partners on the City’s health plan as of January 1, 2020 to earn the premium incentive discount for 2021. Everyone on the City’s health plan as of as of January 1 gets the lower premium rate for Plan Year 2021.
Thanks to everyone who already completed their incentive requirements prior to the COVID-19 outbreak.
IMPORTANT NOTE: There is a change in our premium incentive dates for Plan Year 2022. The new premium incentive program dates are September 1, 2020 – August 31, 2021. Plan members must complete the preventive exam and biometric screening prior to or by August 31, 2021 in order to earn their premium incentive rate for 2022. This also applies to spouses and domestic partners on our health plan.
