Knowing your health numbers – blood pressure, cholesterol – is important to your wellbeing journey. The City of Plano will host onsite biometric screenings at various locations in May! Schedule appointment now.  

To qualify for our Connect4Health (C4H) Premium Incentive Program for 2022, choose from these options:

1. Go to uswellness.com/Plano to schedule an onsite biometric appointment. 
2. If you are not able to attend one of the onsite screenings, you have the option to request a lab voucher for an offsite screening at LabCorp. 
3. A third option is to have your primary doctor complete a biometric screening form during your next visit. 

Biometric screenings include total cholesterol, HDL, LDL, triglycerides, glucose, blood pressure, BMI, waist circumference, body composition and A1C for qualified participants. You’ll receive test results immediately and have the opportunity to discuss them with a trained health care professional onsite.

NOTE: By completing a biometric screening, you will have satisfied 1 of 2 steps for earning your health insurance premium discount in 2022! This applies to employees, pre-65 retirees, spouses and domestic partners who are on the City’s medical plan as of January 1, 2021. 

Remember, the deadline for completing the steps is AUGUST 31, 2021.
