Use Your Nurse Care Manager Resources through Communitas

Should you or an insured family member experience a major illness or injury, the last thing you need is the stress of dealing with doctors and many bills. Consider using one of your benefits, Communitas.  

The services of highly-trained, compassionate RN Certified Care Managers are at your disposal. They bring clinical expertise to the table, as well as skills in navigating billing matters. With Communitas care managers’ assistance, you understand your physician’s instructions better and you don’t experience roadblocks in getting the care you need.  

Nurse care managers provide immediate onsite assistance and no protected health information is ever shared with the City. This is a free benefit for employees, dependents and pre-65 retirees on our health plan. Meet your nurse care managers:
