Don’t wait to schedule your annual exam and be eligible for Connect4Health (C4H) 2023 premium incentives. Getting in to see your doctor may take a while. With an August 31 deadline to meet requirements, it’s a good idea to schedule doctor appointments now.

The wait time to see a physician has increased. In some areas, the wait can be a month or longer. At the core of the scheduling dilemma is a shortage of doctors and an increase in the number of people with health insurance.

Consider this a friendly C4H reminder. If you haven’t completed the two requirements to save money on 2023 benefit costs, make your appointment NOW. Earn your premium incentive by completing two tasks between September 1, 2021 and August 31, 2022:

1. Preventive Exam
   • Choose one of the following: 
     • Annual Exam
     • Well-Woman (OBGYN visit)
     • Colonoscopy 
     • Mammogram
2. Biometric Screening
   • Download your biometric screening form at planotx.uswellness.com and take it to your doctor’s visit OR;
   • Download a LabCorp voucher at planotx.uswellness.com and take it to a facility near you

The C4H premium incentive program applies to all employees, spouses and domestic partners enrolled in the City’s medical plan as of January 1, 2022.
